This Crisis Plan PDF is designed for use by both clients and practitioners as an easily accessible and practical tool to identify warning signs, strengthen protective factors, create a safer environment, and outline the supports and strategies that can be used during times of crisis.

Print and complete this Crisis Plan when you are feeling calm and clear-headed (or with the support of a therapist), keep it somewhere easily accessible, provide a copy to trusted supports, and refer to it whenever you notice early signs of distress, triggers, escalating emotions, thoughts of self-harm, suicidal ideation, or other indicators that you may be moving toward a crisis.
